Objetivos
- Seleccione la cirugía apropiada
- Evitar cirugías innecesarias
- Evite embarcarse en una cirugía que esté más allá del alcance de un cirujano
Tumor primario
- Sitio
- Lateral
- Comisura anterior
- Subglótica (>1cm por debajo del borde libre de la glotis)
- Supraglótica
- Hipofaringe
- Vallecula/bot
- Etapa T
- Movilidad de las cuerdas vocales
- Invasión de cartílago
- Extensión extralaríngea
- A través del cartílago tiroides
- Base de la lengua
- Espacio preepiglótico
- Granel
- Afectación arytenoid
- Disfunción laríngea: Estridor, voz, deglución, aspiración
Nodos cervicales
- Palpar el cuello
- Considera Ultrasonido
Primaria sincrónica (tracto aerodigestivo superior, pulmones)
- Los fumadores actuales/recientes tienen 5-10% de segundo primario
- Examen de historia y consultorio
- Panendocopia
- CXR/CT/MASCOTA
