Caso
Megacolon tóxico
Clínica:
Historia — Este paciente había estado en el hospital durante 5 días después de un ictus agudo. Se desarrolló una infección del tracto urinario secundaria al catéter vesical urinario. El paciente fue tratado con antibióticos y la infección vesical mejoró.
Se completó el traslado a una unidad de rehabilitación. El paciente desarrolló diarrea intratable y se volvió obtundido, febril y parecía muy mal. Se solicitó cirugía general para evaluar al paciente.
Síntomas — Pobre historiador por accidente cerebrovascular y disminución de la conciencia.
Físico — El abdomen estaba extremadamente distendido. El paciente estaba en dolor intenso. Se mejoraron los sonidos intestinales. Había resguardo pero no rebote. La turgencia cutánea fue pobre, lo que sugiere deshidratación.
Laboratorio — El paciente era hipernatremico. La urea y creatinina fueron elevadas. El recuento de glóbulos blancos fue muy alto. Se observó anemia macrocítica leve.

Hallazgos:
El contraste no se pudo administrar por vía intravenosa debido a la mala función renal. Hubo consolidación irregular en las bases pulmonares y pequeños derrames pleurales. El colon estuvo generalmente, masivamente, distendido. Se observó marcado engrosamiento de la pared colónica en la pelvis. No hay evidencia de neumatosis intestinal o neumoperitoneo. No hay evidencia de obstrucción.

Discusión:
Megacolon tóxico es el término clínico para una colitis aguda, tóxica, con dilatación del colon. La dilatación puede ser total o segmentaria. Un término más contemporáneo para megacolon tóxico es simplemente colitis tóxica, ya que los pacientes pueden desarrollar toxicidad sin megacolon.
Las características del megacolon tóxico (colitis tóxica), una condición potencialmente letal, son la dilatación colónica no obstructiva y los signos de toxicidad sistémica.
Cualquiera de los siguientes puede predisponer a un individuo al megacolon tóxico: deshidratación, estado mental alterado, anormalidad electrolítica o hipotensión.
Se pensó que la TM (TC) era una complicación solo de la colitis ulcerosa. De hecho, la TM (TC) puede complicar cualquier número de cólitidos, incluyendo inflamatorios, isquémicos, infecciosos, radiantes y pseudomembranosos. Tres de los siguientes factores apoyan el diagnóstico de un componente tóxico: Fiebre (>38 °C), taquicardia (>120 latidos/min), leucocitosis (>10.5 x 10 3/µL) o anemia.
Los hallazgos por imágenes pueden incluir:
- Evidencia de dilatación colónica — más de 6 cm de diámetro del colon transverso, y/o 10 cm de diámetro del ciego.
- Impresión del pulgar, engrosamiento de la pared intestinal que se proyecta hacia el lumen (especialmente el colon)
- La perforación puede ser focal, contenida o diseminada.
- Se puede observar neumatosis intestinal (burbujas de gas en la pared intestinal)
- El gas en el sistema venoso portal es una complicación poco frecuente y muy grave.
