Mieloperoxidasa (MPO)
MPO es una enzima que se encuentra en los gránulos primarios de todos los granulocitos y monocitos y no está presente en los linfocitos. MPO es útil para diferenciar entre blastos ALL y AML. 2,3

Granulocitos, Mieloblastos, Varillas Auer: Positivo
Monocitos: Negativos a positivos débiles
Linfocitos: Negativos
Sudán Negro B
Manchas de lípidos presentes en los gránulos primarios y secundarios de granulocitos y lisosomas monocíticos. Similar a MPO, es útil para diferenciar entre AML y ALL pero es menos específico. 1-3
Resultados: 1-3
Granulocitos, Mieloblastos: Positivos
Monocitos: Negativos a débilmente positivos
Linfocitos: Negativos
Esterasas inespecíficas (alfa-naftil acetato esterasa)
Tinción enzimática que se utiliza para diferenciar granulocitos de monocitos. La tinción se considera inespecífica debido a que otras células también se pueden teñir. 1-3
Resultados: 1-3
Monocitos: Difusamente positivos (la positividad puede ser inhibida por fluoruro de sodio)
Granulocitos: Negativos
Linfocitos: Negativos (Excepto los linfocitos T que muestran positividad focal)
Esterasa específica (cloroacetato esterasa)
Tinción enzimática que es específica para granulocitos. 1-3
Resultados: 1-3
Granulocitos (Neutrófilos), Mieloblastos, Varillas Auer: Positivo
Monocitos: Negativos a positivos débiles
Schiff de ácido periódico (PAS)
PAS tiñe compuestos relacionados con glucógeno 3 PAS es útil para la identificación de células linfoides.
Resultados: 1
Eritroblastos leucémicos: Positivos (los eritroblastos normales no son positivos)
Linfoblastos: Positividad de bloqueo
Fosfatasa alcalina leucocitaria (LAP)
LAP es una enzima presente en los gránulos secundarios de neutrófilos y no presente en eosinófilos o basófilos. La LAP es útil para distinguir entre la leucemia mielógena crónica (LMC) de otras afecciones que muestran un aumento de los recuentos de leucocitos. 1
Resultados: 1
CML: Puntuación baja en LAP
Reacciones leucemoides: Puntuación LAP alta
Fosfatasa ácida
Enzima que está presente en los lisosomas de leucocitos normales. 1
Resultados: 1
Células T ALL: Positivo
Células pilosas: Positivas
Las células pilosas también muestran positividad para la fosfatasa ácida resistente al tartrato (TRAP) mientras que otras células serían inhibidas por TRAP. 1
Desoxinucleótidil transferasa terminal (TdT)
La TdT es una ADN polimerasa que se encuentra en células inmaduras. 1 Los resultados son útiles para identificar leucemias linfoblásticas.
