La severidad de una alcalosis respiratoria está determinada por la diferencia entre el PCo 2 real y el PCo 2 esperado
El PCo 2 real es el valor medido a partir de los resultados de gases en sangre.
Si no se presenta trastorno metabólico ácido-base, se toma como punto de referencia un PCo 2 de 40 mmHg (es decir, el PCo 2 esperado).
Si se presenta un trastorno metabólico, la compensación respiratoria producirá un nuevo valor de referencia de PCo 2 para su comparación. El PCo 2 esperado puede estimarse utilizando la fórmula de la Sección 5.5 (para acidosis metabólica) o la Sección 7.5 (para alcalosis metabólica).
